Course Synopsis & Learning Objectives

Dr. Joseph Kan: “Immediate Tooth Replacement and Esthetics Zone Tissue Management: Lessons learned over 26 Years ”

Synopsis:  Achieving anterior implant esthetics is a challenging and demanding procedure. Creating implant restorations with harmonious gingival contour that emulate nature is a fusion of science and art. Understanding the biologic and physiologic limitations of soft and hard tissue will facilitate predictability in complex esthetic

situations. This presentation will focus on current implant treatment philosophies and methodologies for replacing currently missing teeth and the management of patients who will be losing a tooth or teeth in the esthetic zone. Emphasis will be placed on diagnosis and treatment planning, interdisciplinary treatment planning, and management of soft and hard tissue for anterior implant esthetics.

Learning Objectives:

1. Prognostic keys for predictable implant esthetics

2. Understanding Soft and hard tissue Biology

3. Immediate Tooth Replacement: What we learned over the past 26 years?

4. Root Shield: What we learned from 13 Year of Experience?

5. Split Bone Plate Grafting Technique; What we learned over the past 12 Years?

6. Bone and Soft tissue graft concepts

7. Implant Papilla management

8. Management of surgical and prosthetic complications

9. Provisionalization (contour for optimal gingival esthetics)

Dr Dan Cullum: - Immediate Molar Implants and Minimally Invasive Grafting – “Evidence and Excellence in Clinical Outcomes”

Synopsis:  Ideal implant positioning with adequate quality and quantity of hard and soft tissue is imperative to long term biological and clinical success. Immediate implant placement will be presented along with current evidence-based techniques for immediate management of multi-rooted teeth in both maxillary and mandibular extraction sites. Advances in implant design, CBCT diagnosis and digital planning, dynamic navigation, defect management and adjunctive crestal sinus elevation (at maxillary extraction sites} offer patients predictable immediate implant reconstruction. These techniques along with use of biologics promote a faster healing interval with minimal discomfort. We will encourage appropriate pathways for progressive skill development and to avoid complications.

Learning Objectives:

1. Apply CBCT diagnosis and digital planning in technique selection to avoid complications.

2. Understand the role of Dynamic Navigation to advance ideal implant size and surgical positioning.

3. Identify defects appropriate for reconstruction with immediate implant techniques.

4. Understand minimally invasive approaches, materials & biologics to optimally restore bone and soft tissue.

Dr. Dan Cullum & Dr. Kevin Hintz – Segmental Maxillary Full Arch Treatment: Blended Digital and Analog

Synopsis: We will discuss "evidence based" full arch reconstruction with immediate implant/provisional restorative protocols with eight implants with no bone removal and four to six implants with cross arch stabilization. Staged and immediate protocols using dynamic navigation for maxillary full arch reconstruction will be presented. Discussion will include optimizing analog and digital workflows, critical prosthetic design requirements for surgical site assessment, preparation and implant positioning for long term bone stability. Pearls and protocols will be reviewed that have demonstrated predictable long term clinical success that will enhance patient centric care and referral relationships.

Learning Objectives:

1. Understand Full Arch Treatment concepts to prioritize planning and treatment      sequence/strategy to meet patient concerns.

2. Apply segmental maxillary reconstruction techniques to avoid alveolar bone resection.

3. Incorporate optimal digital and analog protocols for full arch immediate implant reconstruction into your practice.

4. Identify the role of Dynamic Navigation, CT generated/CAD for accuracy and efficiency in surgical and prosthetic full arch reconstruction.

Dr Robert Pauley JR. “CBCT, IOS & Implants: Steps for Predictable Surgical and Prosthetic Implant Workflow”

Synopsis:  Clinicians at times question what data, in which particular format and the sequencing of the transmission of this information to dental laboratory technicians is needed to begin the development of a prosthetic driven implant plan for both single unit and full arch implant restorations.  The importance of developing and utilizing a digital dental workflow will be discussed and validated with evidence based literature with respect to accuracy, maximization of clinical chair-time, lab collaboration, development of predictable workflow;  The objective of this presentation is to allow clinicians to visualize how a digital dental workflow can be utilized to document the patient’s existing condition from which the dentist & lab technician can collaborate on the prosthetic design creating in essence an ideal Cosmetic Blueprint through which virtual implant placements can be planned and ultimately utilizing dynamic navigation to place the implants in the proper alignment to optimize surgical and prosthetic results.  At the end of this presentation attendees should be able to apply these principles to their next implant patient.

Learning Objectives:

1. Discuss what data is needed to develop a prosthetic driven implant workflow for an immediate single tooth screw – retained provisional incorporating a rationale derived from evidence – based literature

2. The participants will be shown the collaborative steps needed between the dentist and the lab technicians to design and fabricate the immediate provisional

3. Finally, the participants will have been shown the steps necessary to utilize an intraoral scanner and digital imaging software workflow to allow the design and fabrication of a final screw – retained implant crown

4. Ultimately the participants should be able to discern points from the paper discussion back to their offices and immediately implement the workflow into their protocols.

Dr Brandon Kofford: Clinical Application of Smart Denture Conversion at Surgery and use of Digital Workflows for Predictable Final Delivery

Synopsis We will review full arch implant dentistry to simplify our analog approach to FP3 immediate load prosthetics using Smart Denture Conversions. The role and impact of multi-unit abutment selection will be discussed to achieve optimal design and convex intaglio contours. The use of stock dentures in full arch fixed prosthetics will bepresented along with techniques to enhanced accuracy of intra-oral scanning and digital records in full arch fixed treatment.

Learning Objectives:

1. Learn the Smart Denture Conversions protocol on bench-top models

2. Learn how to digitize provisional prosthetics and accurately capture MUA positions  using intra-oral scanners

3. Learn the benefits and technique to complete definitive restorations with stock arches

Dr Robert W Emery: Dynamic Navigation: Immediate and Full Arch with Photogrammetry for Immediate Provisionalization

Synopsis:  Dynamic navigation for the placement of dental implants has been shown to be accurate, precise, and efficient. Recent advances in hardware and software have enhanced presurgical planning, navigation surgery and fabrication of a provisional. Any CBCT can be used for navigation at single sites to full arch cases using anatomic registration. After implant placement the navigation unit can be used to take final impressions using photogrammetry techniques. This program will review these new developments to improve efficiency in full arch immediate load implant cases.

Learning Objectives:

1. Review the literature related to dynamic navigation and its uses in oral and maxillofacial surgery.

2. Review the latest techniques for the placement of immediate dental implants using dynamic navigation and dynamically guided prosthetics for single unit to full arch cases.

3. Review the principles of photogrammetry and take a full arch impression using dynamic navigation.

Dr Michael Block:  Navigation and Digital Work-flow in Immediate Implant Reconstruction

Synopsis:  This presentation will demonstrate the workflow, treatment planning, surgical technique, and long term follow-up on using navigation to optimize implant treatments. A focus will be on single tooth restorations in the maxilla followed by full arch restorations.  Methods will be shown that decrease the need for a flange with a natural appearing final restoration that can be maintained long term. Cases with follow up ranging from 10 to 40 years will be shown demonstrating how relatively simple methods can result in excellent long term soft and hard tissue preservation and health. The goal is to recreate natural appearing teeth with excellent access for maintenance.

Learning Objectives:

1. Understand how to plan a case with no bone removal resulting in full arch restorations single tooth replacements

2. Understand when to graft sites and stage the implant placement to optimize implant position for a flangeless natural appearing restoration.

3. Understand options for patients who need simultaneous sinus grafts when placing implants

4. Understand the use of materials to deliver growth factors to grafted sites when bone defect grafting is necessary.
